The Changing Face of Eye Care

COVID-19’s rapid sweep across the country has forced optical practices to make rapid clinical management decisions. Some optometrists temporarily shuttered their businesses due to the pandemic, while others began to offer emergency appointment services and telehealth.

As mandatory restrictions begin to lift in many locations, optometrists are beginning to open their doors for routine care. But this time around they will implement strict social distancing guidelines and take unprecedented precautions to limit the spread of infection.

Some of the Changes You Should Expect to See At Our Carlsbad Eye Clinic

1) Signage throughout the office spelling out new steps and protocols to ensure maximum safety for staff and patients alike.

2) Social distancing will be the new norm. Packed waiting rooms will be a thing of the past. Instead, clinics will be spacing out seating to reduce capacity and scheduling in longer intervals to minimize patient interactions. Some clinics may ask patients to wait in their cars until they receive a text message from the office stating that they can come in.

3) Certain practices will require appointments for individuals to see and try on the array of frames and sunglasses at the dispensary. Bookings will be in 15-20 minute increments, accessed by one individual at a time.”
4) Methods will be introduced to decrease the number of surfaces a patient touches. This will include leaving the clinic’s front door open (or replacing it with a motion-activated door), facilitating cashless payments, and encouraging patients to fill out registration forms online.

5) Patients who aren’t feeling well or who have been in contact with someone who is ill will be asked to reschedule their appointment two to three weeks in the future.

6) Measuring one’s temperature at the entrance will become commonplace — this goes for both staff and patients. Though not the most reliable screening tool, as those who are asymptomatic can still spread the virus, it will identify some people who aren’t well. Anyone registering 100.4° or above will be sent home.

7) There will be more time between appointments, to allow the staff to thoroughly clean and disinfect before and after each patient’s visit.

8) Many eye practitioners will be wearing safety goggles and face masks, particularly during any up-close contact with the patient. Patients may also be asked to wear masks.

9) Individuals with suspected ocular infections will be put in a special containment area.

10) Practices will frequently wipe down any patient area, including chairs, counters and doorknobs. Every exam room will be completely disinfected between appointments. In the dispensary, frames will be promptly disinfected after patients touch them.

This may sound obvious, but keeping hydrated is a crucial component in managing DES. Since the eyes are part of a larger system (the body), dehydration will affect them as well. Try increasing your water intake to see if symptoms clear up on their own. You’ll want to stick to drinking water and avoid excess caffeine or alcohol, as these can dehydrate the body. Many health authorities recommend drinking about 2 liters of water per day to stay hydrated.

3. Eat More Fish or Flax seeds

Healthy tears are made of water, oil, and mucus, and any imbalance of any of these components can result in DES. Including the recommended amount of Omega-3 fats in your diet can help support healthy tear production. The U.S Department of Agriculture recommends consuming at least 250mg of Omega-3 fatty acids daily.

4. Shield Your Eyes

Wearing protective eyewear, such as wraparound sunglasses or UV blocking lenses, can shelter your eyes from harmful debris and irritants in the air. Breezy weather can also contribute to dry eyes, as the wind causes the eye’s tear film to evaporate faster than the normal pace.

Use a humidifier to counteract the drying effects of indoor heaters or generally dry air.
Point car vents or indoor heaters away from your face when the heat is on. Try to keep your distance from direct sources of heating, especially if they blow out the heat.
Drink a lot! Hydrating your body will also hydrate your eyes.
Protect your eyes outdoors with sunglasses or goggles – the bigger the better! Larger, even wrap-around glasses as well as a hat with a wide brim will keep the wind and other elements out of your eyes. If you wear goggles for winter sports, make sure they fit well and cover a large surface area.”

“Soothe dry eyes using a warm compress and never rub them! Rubbing your eyes will increase irritation and may lead to infection if the hands are not clean.
Give your eyes a digital break. People blink less during screen time which is why extensive computer use can lead to dry eyes. Follow the 20/20/20 rule by taking a break every 20 minutes to look 20 feet away for 20 seconds and make sure you blink!
For contact lens wearers: If you wear contact lenses, dry eyes can be particularly debilitating as the contact lenses can cause even further dryness and irritation. Contact lens rewetting drops can help your eyes feel better and may also allow you to see more clearly. Not all eyedrops are appropriate for use with contact lenses, so ask your optometrist which eyedrop is compatible with your contacts and cleaning solution. If rewetting drops don’t help, consider opting for glasses when your dry eyes are bad, and speak to your optometrist about which brands of contact lenses are better for dry eyes. Many people find dry eye improvement when they switch to daily single use contact lenses.”

Chronic Dry Eyes or Dry Eye Syndrome

Dry eye syndrome is a chronic condition in which the eyes do not produce enough tear film, or do not produce the quality of tear film needed to properly keep the eyes moist. While winter weather can make this condition worse, it is often present all year round. Regular exercise is an essential component of overall health and wellness. It is proven that exercise reduces sickness and disease; it increases strength, immunity, and mental health; and it also helps regulate bodily functions and maintain a healthy weight. Research shows that exercise can lower our risk of chronic conditions such as high blood pressure, heart disease, diabetes, diabetic retinopathy and diabetic macular edema, as well as other eye diseases such as glaucoma, cataracts and wet age-related macular degeneration (AMD).

Whereas, a sedentary lifestyle increases the risk of these diseases and of vision loss, studies show that even moderate exercise at least three times a week can improve the prognosis of the above-mentioned chronic illnesses and reduce the risks of developing vision threatening eye diseases.

Inactivity is an even higher risk factor if you have other co-factors for developing eye diseases, including: a family history, previous eye injury or surgery, diabetes, high blood pressure or very high myopia. A combination of healthy lifestyle habits which include regular exercise and a nutritious diet and tending to your mental and emotional well-being can reduce these risks significantly.”

Tips for Incorporating Physical Activity Into Your Day

Make it a priority. Schedule your exercise time into your day as if it is a non-negotiable appointment. Find the time of day that works best – for some that is early morning and for others late at night. Work your way up to a half hour at least three times a week.
Be realistic. You don’t need to become a fitness expert to experience the benefits of exercise. Walking, yoga, swimming, even dancing around the house are all options for staying fit. Find a type of exercise that you love so you will enjoy working this habit into your life.
Just move. Find ways to move your body throughout your day. Park your car a little further away from the mall entrance, take the stairs instead of the elevator or walk or bike to work. Remember, every little bit of movement helps.
Find something you enjoy. Often finding the right exercise is a good stress reliever, and reducing stress will also reduce risk of many chronic diseases.
It’s never too late. Exercise for the elderly can be a challenge especially during the cold winter months, when many seniors can’t get out of the house due to the weather. Even walking up and down the stairs in the house or following an exercise video can be helpful to keep from being sedentary.”

This can cause a consistent lack of sufficient lubrication and moisture on the surface of the eye, known as dry eyes.

Normally, the eye constantly lubricates itself with tears by producing them at a slow and steady rate, keeping itself moist and comfortable. Usually these tears consist of three layers, an oily, a watery, and a mucus layer. Each layer has a specific role in lubricating your eyes. The oily layer is outermost. It’s main purpose is to slow evaporation of the tear. The watery layer is in the middle. This makes up the majority of what a person normally thinks of as tears. This layer cleans the eye and helps to wash away small foreign objects and particles. The inner layer consists of mucus. This mucus allows the watery layer to stick to the eye and spread evenly over the eye in order to keep it lubricated. In a person with dry eyes, either hormonal changes, side effects from medication or some other factor causes the eye to either not produce enough tears, or leave out parts of the tear that make proper lubrication possible.

Although dry eyes are not always curable, your optometrist may prescribe artificial tears to help with some of the symptoms. Artificial tears are lubricating eye drops that may help with dry, scratchy feeling eyes. Different artificial tears work in different ways. Some help replenish parts of the tear that your eyes are not producing on its own, others help to produce more tears overall. Your eye doctor will assist you to choose which will help you most. Dr. Osborne cautions, however, “These artificial tears should not be confused with eye drops that are advertised to ‘get the red out.’ These eye drops may indeed reduce the appearance of redness in your eyes, but this is accomplished by making the blood vessels in your eyes smaller rather than actually lubricating your eyes. As such, these drops can sometimes actually make your symptoms worse.

Dr. Osborne, of Douglas Osborne, OD in Vista, California explains, “Symptoms of dry eyes include stinging or burning in the eyes, scratchiness, and excessive irritation from smoke or wind. Although it may sound counter-intuitive, the eyes’ response to the consistent irritation caused by dry eyes may also cause a person to experience excessive tearing. In this case, the eye is attempting to flush and lubricate itself by producing more tears, but is unable to do so successfully due to the rate of evaporation or inability to spread the tears properly.”, Dr. Osborne, Vista eye doctor.

Some cases of dry eyes are seasonal, such as those which occur as a result of cold, dry winter air. In this case, your eye doctor may recommend wearing sunglasses or goggles when outdoors to reduce your eyes’ exposure to the sun, wind and dust. For indoors, your optometrist may recommend an air cleaner and humidifier to take dust out of the air and add moisture to air which is too dry.
One should also be aware that if you wear contacts, some eye drops require you to take them out before using the drops and wait 15 minutes or more before reinserting your contact lenses.”
Studies have also shown that nutrition may have a part in helping to relieve some symptoms of dry eyes. Your eye doctor may recommend nutritional supplements such as omega-3. Good sources of omega-3 fatty acids are cold-water fish, cod, herring and salmon, as well as flaxseed oil. Mild dehydration can make symptoms worse too, so be sure to drink plenty of water, 100 percent fruit and vegetable juices and milk.
